Job summary

Practice Plus Group is seeking an Out of Hours General Practitioner to provide patient-centred urgent care during evenings, weekends and bank holidays. You will work within a multidisciplinary team, delivering high-quality care across base visits and home visits while maintaining safety and continuity of care.

Qualifications

• Fully qualified GP with GMC registration and UK licence to practise.
• Experience in Out of Hours setting is essential.
• Experience using Adastra for patient records is essential.
• Strong clinical assessment and ability to manage diverse caseload.
• Excellent communication and interpersonal skills; able to work independently and in a team.
• Flexible working hours including evenings, weekends and bank holidays.

Responsibilities

• Provide urgent care assessments and management for patients outside normal hours across base and home visits.
• Manage a varied caseload including acute illnesses, minor injuries and chronic condition exacerbations.
• Record consultations accurately using the Adastra system.
• Collaborate with nurses, paramedics and admin staff to ensure continuity of care.
• Make referrals to secondary care and ensure patient safety through clinical decision-making.
• Participate in audits, governance activities and CPD.

• Providing comprehensive clinical assessments and management for patients requiring urgent care outside of normal surgery hours across base working and home visitations.
• Managing a varied caseload including acute minor illnesses, injuries, and chronic condition exacerbations.
• Utilizing the Adastra clinical system to accurately record consultations and clinical notes.
• Working collaboratively with healthcare colleagues, including nurses, paramedics, and administrative staff, to ensure continuity and quality of patient care.
• Making appropriate referrals, liaising with secondary care services, and ensuring patient safety through effective clinical decision-making.
• Participating in clinical audits, governance activities, and continuous professional development.

• Fully qualified General Practitioner with GMC registration and relevant UK licence to practice.
• Previous experience working in an Out of Hours setting is essential.
• Proven experience using the Adastra clinical system for patient record management.
• Strong clinical assessment skills and the ability to manage a diverse and often complex caseload.
• Excellent communication and interpersonal skills with the ability to work both independently and as part of a team.
