Practice Plus Group is seeking an experienced Advanced Nurse or Clinical Practitioner to join our Out of Hours team, delivering high-quality patient care when GP services are closed.

Qualifications

• Experience as Advanced Nurse or Clinical Practitioner.
• Ability to work flexibly in Out of Hours care.

Responsibilities

• Assess and treat patients in Out of Hours settings, providing urgent unscheduled care.
• Refer or escalate to GP services when appropriate and ensure safe handovers.

We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you.

In OOH we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU.
