Job description

The Red House Surgery seeks a highly motivated, experienced Advanced Clinical Practitioner to join our dedicated, multidisciplinary clinical team providing a high standard of care for our practice population of 22,000 patients.

Our team is friendly, approachable, and supportive, ensuring a pleasant working environment.

Main duties of the job

The ANP role involves assisting the Duty Team with daily triage, telephone consultations, face-to-face consultations, and a daily home visit as required.

The successful applicant will demonstrate critical thinking skills to make safe clinical decisions, following practice policies and procedures. They will work collaboratively with practice colleagues to provide a high standard of care and meet patient needs.

We are a well-established, well-organised, supportive practice in Cambridge, serving 7,800 patients. We have three partners and a team including salaried doctors, retainer doctors, Physician Associates, nurses, GPAs, physiotherapists, clinical pharmacists, social prescribers, and social prescribers.

We have a strong interest in teaching and training medical students and GP trainees. We play an active role in Cantab PCN.

We have consistently high QoF achievement and are rated good by CQC.

The three partners enjoy a close working relationship with the Practice Business Manager, Practice Manager, and each other. We regularly meet to ensure effective communication and collaboration to meet our goals. Staff well-being is a priority.

Job responsibilities

The core responsibilities of the Advanced Practitioner include, but are not limited to, the following:

  1. Providing efficient and effective medical triage as a member of the duty team.
  2. Managing patients presenting with a range of acute and chronic medical conditions, providing expert advice in practice or at patients' homes for housebound patients.
  3. Implementing and evaluating individualised treatment plans for chronic disease patients.
  4. Identifying, managing, and supporting patients at risk of developing long-term conditions to prevent adverse health effects.
  5. Providing advanced, specialist care in accordance with clinical evidence, NICE guidelines, and the NSF.
  6. Requesting and interpreting pathology and other test results as necessary.
  7. Maintaining accurate clinical records in compliance with legislation.
  8. Ensuring effective use of read codes and maintaining chronic disease registers.
  9. Prioritising health issues and intervening appropriately.
  10. Supporting the team in clinical emergencies.
  11. Recognising, assessing, and referring patients with mental health needs.
  12. Practising as an extended and supplementary prescriber within guidance.
  13. Supporting patients in medication use, reviewing annually as needed.
  14. Contributing to practice targets (QoF, IIF) in line with guidance.
  15. Liaising with external services to support vulnerable patients.
  16. Delegating clinical responsibilities safely within scope of practice.
  17. Supporting safeguarding matters per policies.
  18. Understanding policies related to substance abuse and addiction, with appropriate referrals.
  19. Delivering opportunistic health promotion where suitable.
Person Specification
Qualifications
  • Active registration with the Nursing and Midwifery Council (NMC).
  • Level 7 MSc qualification in clinical assessment.
  • Independent prescriber.
  • Experience working in primary care.
  • 2-3 years of clinical experience.
  • Experience in disease management.
  • Experience using SystmOne.
