Advanced Clinical Practitioner - Living Well & Care Coordination Centres, St Albans

This role aims to improve clinical practice and patient outcomes across RGP Living Well and Care Coordination Centres by fostering a culture of learning, quality improvement, and innovation. It involves delivering direct patient care using advanced clinical expertise, particularly in the palliative care sector, and supporting colleagues to ensure best practices and optimal patient outcomes.

Key responsibilities include:

  1. Leading and supporting the operational delivery of the RGP strategy into clinical practice, ensuring timely achievement of the annual Patient Services work plan.
  2. Delivering evidence-based practice to improve patient and carer outcomes across the entire patient pathway.
  3. Monitoring safe and responsive patient care, leading multidisciplinary care planning meetings and case reviews for effective care coordination.
  4. Embedding the RGP Quality and Assurance framework into daily practice, ensuring clinical policies and guidelines are followed and audit findings are addressed.
  5. Managing patient-related incidents proactively, fostering a culture of transparency and continuous improvement, and recommending practice or process changes as needed.
  6. Overseeing the development needs of the clinical workforce, collaborating with Practice Educators to ensure adequate training, supervision, and mentorship.
  7. Working with the Learning & Development team to make Palliative Care an attractive workplace, including developing induction and mentorship pathways and external placements.
  8. Mentoring and coaching nursing and AHP staff to enhance clinical decision-making and optimize workforce skills.
  9. Staying informed on national and local palliative care initiatives to drive service redesign and improve patient experience.
  10. Collaborating to enhance early diagnosis pathways and engaging with NHS and Social Care leads to develop joint initiatives.
  11. Exploring opportunities with various stakeholders to utilize ACP leadership effectively.
  12. Being proactive in research activities.
  13. Leading as the Medical Devices Safety Officer (MDOS), chairing the Patient Safety interest group, and reporting to the RGP Patient Services Safety group.

Requirements:

  • Right to work in the UK.
  • Ability to work 37.5 hours per week; part-time considered.
  • Experience in palliative care and clinical leadership.
