Main duties of the job
The role aims to enhance clinical practice and patient outcomes in RGP Living Well and Care Coordination Centres through a culture of learning, quality improvement, and innovation. Using advanced clinical expertise and extensive knowledge of palliative care, the practitioner will deliver direct patient care and support colleagues to maintain and improve standards.

Job responsibilities
Applicants should demonstrate abilities in:
- Leading and supporting the operational delivery of RGP strategy into clinical practice, ensuring timely achievement of the annual Patient Services work plan.
- Delivering evidence-based practice to improve patient and carer outcomes across the patient pathway.
- Monitoring safe, responsive patient care, leading multidisciplinary care planning and case reviews.
- Embedding quality assurance frameworks into daily practice, ensuring policies and audit findings are implemented.
- Managing patient-related incidents and fostering a culture of transparency and continuous improvement.
- Overseeing workforce development needs, working with Practice Educators on training and mentorship programs.
- Making palliative care attractive through robust induction, mentorship, and external placements.
- Mentoring and coaching clinical staff to enhance decision-making and skill utilization.
- Staying informed of national and local palliative care initiatives and driving service improvements.
- Collaborating to enhance early diagnosis pathways and create joint initiatives with NHS and social care partners.
- Exploring opportunities for leadership and research in clinical practice.
- Leading on the Medical Devices Safety Officer role and reporting on patient safety issues.

Essential
- Regulated healthcare qualification
Additional Criteria
Essential
- Master’s level or willingness to undertake the ACP apprenticeship
- Independent Non-medical Prescriber and assessment qualification or willingness to undertake
- Experience in palliative or long-term condition care (Band 7)
- Advanced communication skills training
