We are looking for Administrators to join our Integrated Care Coordination for Urgent Care service based in Hanley, Stoke-on-Trent to provide administration support and receive and action referrals. The primary aim of the Integrated Care Coordination (ICC) for Urgent Care is to be a single point of access to support patients to remain in their own home to prevent unnecessary admission to acute care and to facilitate collaborative Multidisciplinary Team (MDT) management for patients closer to home. You will play an integral part of the team by providing effective and efficient call handling, You should be highly skilled in administration and demonstrate keen self-management skills to enable them to provide an effective and efficient high-quality service. You will provide a varied range of administrative duties and act as a focal point for communication and contribute to achieving key objectives and outcomes of the service. You will be required to cover the 7 day period, 365 days and therefore weekend and out of hours working is required. You will be dealing with referrals from GP's, WMAS, Health Care Professionals, Social Care Practitioners, and other professionals by telephone or other electronic formats and providing non-clinical advice, information and guidance to healthcare professionals and social care practitioners, to deal with all enquiries and communicate effectively on behalf of the Team. Excellent interpersonal and communication skills (both written and verbal) are essential, as is an ability to organise and prioritise your workload and react quickly to changing situations.
