About You

About The Role

You will support adults with disabilities and support needs at Ambito day services and community locations, including Jubilee library and swimming facilities. Your main task is to lead activity sessions and collaborate with other coordinators to plan and review activities tailored to individual needs.

Key Responsibilities

• Ensure staff are equipped to support individuals in sessions, collaborating with Support Workers and holding regular meetings to review and adapt plans.
• Facilitate interaction and experience of activities, managing sessions, resources, and contributing to person-centred planning and documentation.
• Maintain a focus on individual needs, supporting participation, learning new skills, health and well-being, and personal care as needed.
• Record keeping, attending training and meetings, behaving respectfully, and adhering to policies and practices.
• Create a safe, healthy, and supportive environment, with additional duties as assigned.
