Job summary

Active Care Group is seeking experienced Female Support Workers to join our Northern Case Management client support teams. You will provide dedicated, person-centred care for a young client with acquired neurological damage in home and community settings, including school and transport support.

You will work a mix of waking days, nights and sleeps with a 2:1 day ratio and 1:1 nights, requiring commitment, empathy and strong teamwork.

Qualifications

• Genuine interest in providing person-centred care.
• Training will be provided for complex needs.
• Excellent communication and interpersonal skills.
• Willingness to learn and develop.

Responsibilities

• Cover 24/7 hours in rotation, including holidays and sickness periods.
• Promote and maintain independence for an individual with complex care needs.
• Develop skills while delivering care in client’s home or family home.
• Undertake a range of duties to support the client and work with the care team.
• Work independently and as part of a team to meet high standards of health care.
• Demonstrate reliability, integrity and trustworthiness.
