As a Patient Care Coordinator, you will serve as a primary point of contact for patients and referral sources, helping facilitate onboarding, communication, care coordination, and access to services. This role plays a vital part in ensuring patients receive a seamless, supportive, and positive experience from referral through ongoing care.
The ideal candidate has a healthcare background and is comfortable communicating with patients, providers, caregivers, pharmacies, and community partners. This role involves supporting medication orders, care coordination activities, clinical documentation workflows, and patient communication. Individuals with medical office experience and a willingness to learn may also be successful in this position.
This role goes beyond answering phones or scheduling appointments. The Patient Care Coordinator serves as an advocate, guide, and resource for patients and families while helping ensure a positive and seamless care experience.
