Timmins and District Hospital is seeking a Nurse Practitioner to join the Diabetes Program. The NP will provide advanced clinical care, collaborate with physicians, educators, dieticians, pharmacists and social workers, and educate patients for self-management.
The role includes ordering and interpreting tests, prescribing meds, and coordinating referrals within Ontario standards. Hospital-based, full-time position with opportunities for program development and education.
Reporting to the Director of Clinical Services the NP within the Diabetes Program is a crucial role in the comprehensive care of patients with diabetes, focusing on both treatment and education to empower patient’s self-care to manage their condition effectively. The NP will work collaboratively with the interdisciplinary team of professionals including Physicians, Educators, Dietician, Pharmacist and Social Worker in providing a seamless outpatient journey for all patients presenting to the clinic. The NP will practice to the full NP scope of practice by ordering and interpreting diagnostic tests, prescribing medications, education, monitoring, referring patients when required to other specialists and all other duties as assigned within the CNO Standards of Practice, Nurse Practitioners and policies of the TADH.
