Fort Sanders Regional Medical Center is seeking a Registered Nurse for Transitional Care on a full-time night shift (72 hours per pay period). The facility is part of Covenant Health and offers advanced stroke care and orthopedic services.

The role focuses on coordinating care, patient education, and supervising care teams to ensure high-quality outcomes. Responsibilities include conducting nursing histories, collaborating with the care team, and implementing the care plan while ensuring safety

Qualifications

• Active RN license in Tennessee or eligible for licensure.
• CPR certification required.
• New graduates welcomed within the first year of orientation.

Responsabilités

• Provide professional nursing care within an assigned unit.

Position Summary

Provides professional nursing care within an assigned unit; coordinates care planning with other disciplines.

Responsibilities

• Completes nursing history and initiates patient education.
• Collaborates with interdisciplinary team members.
• Implements plan of care appropriate to diagnosis. Evaluates care outcomes.
• Problem solves and coordinates unit/patient care assignments, with supervision.
• Introduces new employees/students to unit routines.
• Participates on unit-based committees.
• Demonstrates competence in performing critical skills (checklist) to include appropriate delivery of care according to the age-specific needs of the population served.
• Supervises LPNs, Nursing Assistants, HUCs, Techs and sitters.
• Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
• Performs other duties as assigned.

Qualifications

Minimum Education

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience

New graduate, within first (1 st ) year of orientation (move to level II after one (1)Year experience).

Licensure Requirement

Must have and maintain Tennessee State RN license. CPR required.
