Job summary

Cigna Healthcare, based in Europe, seeks a Nurse Case Manager (Afternoon Shift) to support members with complex medical or care-coordination needs. The role is nurse-led and focuses on personalised care plans, advocacy, and coordinating providers to ensure timely, appropriate care. Work Monday to Friday, 12:00 p.m.

to 8:00 p.m. in a hybrid, international environment. The successful candidate will have a Nursing degree, 3–5 years in Case Management, and fluency in English; additional languages

Qualifications

• Bachelor's Degree in Nursing required.
• 3–5 years in Case Management, Clinical Management, or related healthcare setting, preferably international.
• Fluent in English; additional languages (French, Portuguese or German) are strong assets.

Responsibilities

• Advocate for members and coordinate care across providers.
• Assess medical, psychosocial and functional needs; develop personalised care plans.
• Monitor progress, ensure safe transitions of care, and collaborate with physicians and teams.
• Educate members on conditions, treatment options and resources for self-management.
• Deliver specialised clinical programmes with internal and external teams.
• Maintain accurate case records and ensure compliance with standards and policies.
