Jobtailor seeks a Population Health Social Worker to advance patient-centered care in a value-based system. The role focuses on direct patient care, case management, and navigation of community resources to support frail and elderly populations.

You will partner with RNs, providers, and staff to optimize outcomes, advocate for patients, and coordinate care across settings including home health and facilities.

Qualifikationen

• Master’s degree in Social Work REQUIRED
• 2 years' experience in direct patient care and case management REQUIRED
• Experience working with the frail or elderly population preferred
• Knowledge of Medicare, Medicaid and CMS regulations preferred
• Candidates must reside within a 30-minute driving distance of Harrisonburg, VA
• Travel may be required based on patient needs or assigned cases
