Job summary

ChristianaCare HomeHealth in Delaware seeks a compassionate Registered Nurse to provide one-on-one in-home care, fostering independence and high-quality outcomes. You will work day shifts with weekend, holiday, and on-call requirements, building strong patient relationships and coordinating with the care team.

The role emphasizes autonomy, support, and work-life balance, with access to technology that streamlines documentation. Prior home health or med-surg experience is preferred.

Qualifications

• Bachelor of Science in Nursing required.
• ADN holders must obtain BSN within 3 years of hire as per Education Agreement.
• Current Delaware RN license or Compact State RN license.

Responsibilities

• Deliver skilled nursing care and treatments to geriatric patients in homes and community.
• Build trusting relationships with patients and caregivers while promoting health and independence.
• Develop and implement individualized, patient-centered care plans aligned with resources.
• Coordinate care with physicians, caregivers and interdisciplinary team for optimal outcomes.
• Develop and execute discharge plans ensuring continuity of care.
• Perform IV therapy and related nursing functions with training to achieve proficiency within 90 days.

The successful candidate will support patients throughout Wilmington and Newark, Delaware and surrounding communities while working day shift hours, 3 weekends per month - 12 hours on both Saturday and Sunday, two holidays per year, and two on-call shifts per month.

What You'll Do

• Develop and implement individualized, patient-centered care plans that are goal-oriented and aligned with available resources.


• Coordinate care with physicians, caregivers, and interdisciplinary team members to achieve the best possible outcomes.


• Develop and execute discharge plans that ensure continuity of care.


• Perform IV therapy and related nursing functions. New team members will receive training and are expected to achieve IV proficiency within 90 days of hire.

What We’re Looking For

Required Qualifications

• Bachelor of Science in Nursing (BSN) required.


• ADN candidates will be required to obtain a Bachelor of Science in Nursing (BSN) within three (3) years of their hire date, as outlined in the required Education Agreement.


• Current Delaware RN license or Compact State RN license.


• Minimum of one (1) year of RN experience.


• IV proficiency or willingness to complete training and perform IV-related functions.

Preferred Qualifications

• Previous Home Healthcare experience.


• Previous Medical-Surgical nursing experience.

Hourly Pay Range

Hourly Pay Range: $30.95 - $49.52. This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
