UnitedHealth Group is seeking a Registered Nurse Home Health Baylor to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage ongoing outpatient care in Dover, DE.

In this full‑time role, you will supervise LPNs and HHAs, document your work timely, and collaborate with the clinical team to develop and implement plans of care. A valid RN license and CPR certification are required, along with 1+ year of RN experience.

Qualifications

• Current RN license, active in the state(s) you are assigned to work.
• 1+ year of clinical RN experience.
• Valid driver's license with reliable transportation and liability insurance.
• Current CPR certification.

Responsabilités

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures.
• Assist patients in learning appropriate self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Participate in clinical development and continuing education programs.
