NHS Greater Glasgow and Clyde (NHSGGC) invites an HCPC-registered Occupational Therapist to join the West Dunbartonshire Community Hospital Discharge Team. The role involves managing a complex caseload of older and disabled adults, providing assessment, treatment and advice across community settings, including patients’ homes.
The post requires autonomous work within a MDT, contributing to service development, staff supervision, and ongoing CPD.
NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non‑clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.
This is an excellent opportunity for an HCPC-registered Band 6 Occupational Therapist to work within the West Dunbartonshire Community Hospital Discharge Team, managing a complex clinical caseload of older and disabled adults with multiple pathologies. The role involves working autonomously and as part of a dynamic multidisciplinary team to provide assessment, treatment, intervention, and expert advice across community settings, including service users’ homes and other discharge destinations.
The focus of the service is to promote independence, support self-management, avoid hospital admission, and facilitate timely hospital discharge through person-centred care. The post holder will contribute to interdisciplinary assessments, case coordination and service delivery, while also engaging in supervision of staff and students, health promotion, audit activity and service development.
This community-based role requires travel across West Dunbartonshire; therefore, a full UK driving licence and access to a car is essential. Experience of working within a multidisciplinary team and knowledge of community practice, older people’s care, or neurological conditions is desirable.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
