Jobtailor is seeking a Hospice Care Coordinator to receive, coordinate, and track referrals from physicians, hospitals, and community sources. You will gather patient information, initiate consults, and communicate priorities to hospice teams, ensuring timely processing and high-quality documentation.

You will work with Epic EMR and E-Discharge, maintain confidentiality under HIPAA, and develop strong relationships with referral sources, patients, families, and staff while supporting scheduling,

Qualifikationen

• High school diploma required.
• 1-2 years’ experience in a health-related field.
• Excellent observation and problem solving/critical thinking skills.
• Well-developed verbal and written communication skills.
• Essential computer skills to work with EMR.
• Knowledge of Excel.
• Must have a licensed driver with insured vehicle.
• Self-directed and able to work with minimal supervision.
• Strong organization and prioritization abilities.
• Proficiency with Epic and E-Discharge.

• Receive and coordinate hospice and palliative care referrals from physicians, hospitals, facilities, and community referral sources
• Gather accurate patient medical and environmental information to initiate hospice and palliative consults
• Communicate patient priorities and referral information to hospice and palliative teams
• Process referrals accurately and within required timelines
• Schedule, check in, and confirm Nurse Practitioner face-to-face visits
• Coordinate initial evaluations with patients, families, and Nurse Practitioners
• Create denial letters and assist with one-time patient contracts
• Maintain the Hospice Administrator on-call schedule
• Register new hospice patients with the Hospice Pharmacy
• Assist with creating hospice and palliative care EMR records
• Check PMP reports for new hospice admissions and send them to physicians for review
• Manage forms, signatures, scanning, documentation, insurance verification, and admission data entry
• Respond to telephone requests and interpret services to callers
• Use Email, Epic EMR, and other information technologies to process and track referrals
• Develop and maintain relationships with referral sources, patients, families, and staff
• Attend meetings, in-service programs, and quality improvement activities
• Assist with precepting new staff, IDT preparation, and face-to-face visit tracking
• Maintain confidentiality and comply with organizational policies, HIPAA guidelines, and mandatory education requirements

Requirements

• High school graduate required
• 1-2 years’ experience in a health related field
• Excellent observation and problem solving/critical thinking skills
• Well-developed verbal and written communication skills
• Essential computer skills to work with EMR
• Knowledge of Excel
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
• Self-directed and able to work with minimal supervision
• Strong ability to organize and prioritize workload independently
• Proficiency with common office software programs and ability to learn Epic and E-Discharge
• Ability to maintain confidentiality and comply with HIPAA guidelines
• Ability to complete required training and mandatory education
• Associate’s degree in healthcare related field or business is preferred
