Family Health Centers of San Diego is seeking a Case Manager II to provide intensive, therapeutic case management to high-need individuals across our community clinics. Under limited supervision, you will develop formalized care plans, coordinate services, and support clients toward stable housing, health adherence, and access to care.

This full-time role requires a bachelor’s degree in a social science or public health field, 3 years’ experience, and a valid California driver's license.

Formación

• 3 years of work experience providing human services to high-risk, medically underserved populations.
• Bachelor's degree in Social Science, Public Health, or Healthcare Administration.
• Current CPR (BLS) certification for AOD Services Department.
• Valid California driver’s license and reliable vehicle.

Responsabilidades

• Complete accurate documentation and prepare reports.
• Assess clients' health and support needs; develop care plans.
• Provide intensive individual support and coordinate referrals.
• Travel between sites as required.
