Premier Home Health Providers Inc. is seeking a per diem Medical Social Worker (MSW) with an active California LCSW license to lead our social services team and coordinate patient care in home settings.

Manage staff, oversee assessments and care plans, and collaborate on budgets while connecting patients with community resources. Flexible schedule with potential to convert to full-time based on performance.

Formación

• MSW from an accredited program is required.
• Active California LCSW license required.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and trust-building with patients and staff.

Responsabilidades

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers with supervision and growth support.
• Oversee assessment, planning, and evaluation of social services care and ensure compliant documentation.
• Partner with the Executive Director on the department budget and resources.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and identify new patient service improvements.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
