As a Case Manager, you’ll be the first point of contact for prospective tenants, helping them navigate the leasing process, gather key information, and coordinate with property management teams—from home.

Formación

• Proficiency in English and Spanish languages.
• Experience in administrative, coordination, or case management roles preferred.
• Strong MS Excel and Outlook skills.
• Excellent organization and multitasking abilities.
• Compassionate, professional communication skills.

Responsabilidades

• Serve as the primary point of contact between the office and families, building strong and supportive relationships.
• Coordinate and monitor client reassessments to ensure timely scheduling and follow-up.
• Track and report weekly billing trends and assist with operational oversight.
• Conduct monthly care calls with families to review progress, answer questions, and address concerns.
• Support schedule adjustments and resolve case-related issues efficiently.
• Maintain accurate and compliant client documentation and records.
• Collaborate with internal teams to ensure seamless coordination of care and service delivery.
• Help create a positive experience for families through proactive communication and ongoing support.

About the Role

As a Case Manager, you will play a key role in connecting families with care teams, ensuring smooth coordination, timely follow-ups, and an exceptional client experience throughout the care journey.

What You’ll Do

• Serve as the primary point of contact between the office and families, building strong and supportive relationships.

• Previous administrative, coordination, or case management experience preferred.
• Strong proficiency in Microsoft Excel and Outlook.
• Excellent organizational skills with the ability to manage multiple priorities.
• Strong communication and interpersonal skills with a compassionate and professional approach.
• Knowledge of Applied Behavior Analysis (ABA) and behavioral health concepts is a plus.
• Experience working with individuals with Autism Spectrum Disorder (ASD) or healthcare-related environments is preferred.
