Formación

Job Summary:

The Registered Nurse, Directing (DRN) role manages pediatric, palliative, and/or acute medically complex clients with Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF) by directing Personal Support Workers (PSWs), also known as Health Care Technicians (HCT). HCTs are provided specialized training to work with this patient population. Utilizing an innovative care delivery model and electronic documentation technology, the DRN, working remotely, accesses, assesses, and identifies the client’s immediate care needs. Using a collaborative approach, the DRN immediately provides direction to on-site HCTs to deliver direct care, ensuring real-time provision of services.

Responsibilities:
  1. Provide remote assessment and management of care services to clients in their homes.
  2. Provide clinical direction and assign tasks within the unregulated care provider's (PSW/HCT) practice statement, adopting a collaborative approach to patient care.
  3. Maintain accurate client clinical records in digital and paper formats.
  4. Maintain patient assignment within the virtual ward.
  5. Work collaboratively with all team members to ensure all patient care needs are met.
  6. Utilize effective communication in daily huddles and respond to emails/phone calls.
  7. Communicate with outside service providers and Case Managers as appropriate.
  8. Participate as a valued member of the multidisciplinary healthcare team.
  9. Provide clinical leadership, direction, and mentorship within the healthcare team.
  10. Participate in and facilitate a collaborative, team-based environment.
  11. Establish and maintain relationships with local healthcare providers.
  12. Participate in CNO Reflective Practice.
  13. Uphold VON standards and eHomecare specific processes.
  14. Willingness to participate in internal projects and ongoing improvement initiatives.
  15. Demonstrate professionalism in a diverse working environment.
  16. Demonstrate adaptability in managing change in a fast-paced virtual environment.
Qualifications:
  1. Registered and in good standing with the College of Nurses of Ontario.
  2. DRNs must hold a CNA Certificate in Palliative Care or have completed both the "Fundamentals of Palliative Care" and the "Comprehensive Advanced Palliative Care Education (CAPCE)."
  3. Pediatric DRNs must have extensive hands-on experience in pediatric care.
  4. Recent palliative, pediatric, and/or acute medical experience with COPD and CHF patients.
  5. Demonstrated advanced assessment skills.
  6. Experience in gerontology and/or medical-surgical nursing.
  7. Experience working in community-based healthcare systems is an asset.
  8. Competencies in observation, independent assessment, health education, and familiarity with COPD/CHF clinical pathways.
  9. Experience with digital and electronic health records, EMR, and mobile technology.
  10. Proficient nursing skills with the ability to interpret electronic technical information and provide clinical direction.
  11. Critical thinking, problem-solving, and multi-tasking skills are mandatory.
  12. Ability to work independently and in a multidisciplinary virtual team setting.
  13. High-speed internet access, computer skills, and ability to work from home in a private office.
  14. Eligibility to obtain a security screening certificate from the Government of Canada.
  15. Valid CPR C / First Aid certification.
  16. Access to a personal vehicle, with a valid driver’s license and proof of insurance.
  17. Current satisfactory Criminal Records Check required at employment start.
  18. Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
  19. Ability to speak French is an asset in French-designated areas.
  20. Ability to wear Personal Protective Equipment (PPE).
Physical Demands and Working Conditions:

This role requires attention to detail in a dynamic environment, involving physical activities such as lifting, carrying (with proper techniques), bending, reaching, kneeling, and other movements emphasizing good body mechanics. The role involves walking, sitting, standing, and climbing stairs, with some tasks requiring fine hand movements.
