REGISTERED PRACTICAL NURSE (RPN)

Baycrest Adult Day Programs

(Samuel Lunenfeld Mountainview Club)

Job Summary:

The Baycrest Adult day Program is focused on offering older adults a wide range of social, cognitive, physical and spiritual activities. This program is designed for community dwelling seniors with physical frailty as well as moderate to severe cognitive impairments.

Responsibilities include but are not limited to:

• Plans for, delivers and evaluates nursing interventions, including medication administration as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients with moderate to severe cognitive impairment
• Provides leadership and guidance to Program Assistants to meet the clients’ needs and to develop individualized care plans
• Documents plans of care, ongoing assessments, interventions and client progress in the electronic health record
• Provides supervision to clients during mealtimes and responds as necessary to any client medical emergencies

Qualifications include but are not limited to:

• Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required.
• Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
• Previous geriatric experience and experience working with clients with moderate to severe cognitive impairment is preferred
• Excellent interpersonal, communication and problem solving skills
• Basic computer skills – Meditech, Microsoft Word and Excel preferred
