Job summary

YouthLink is recruiting a Wraparound Facilitator to deliver comprehensive wraparound services to youth 12–18 at risk of care or currently in care. You will guide families through strengths-based planning and coordinate a multidisciplinary team to meet each youth’s goals.

The role requires a degree in Social Work or related field, 2 years’ experience with youth, and a commitment to equity and trauma-informed practice. Some local travel and occasional evenings may apply.

Qualifications

• Undergraduate degree in Social Work, Psychology, or related field.
• 2 years’ experience with youth and families; trauma-informed approach.
• Experience with marginalized youth and diverse populations.
• Ability to work in a multidisciplinary team and coordinate services.

Responsibilities

• Provide Wraparound services for youth 12–18 at risk of entering care or in care.
• Coordinate crisis/safety plans and team of supports.
• Develop and implement Wraparound plans with families; advocacy and referrals.
