Job summary

YouthLink is seeking a part-time Diversion Case Manager to support youth at risk of homelessness with housing options, case planning, and coordinated services. The role emphasizes strengths-based, equity-informed care and collaboration with partner agencies.

20 hours weekly, starting ASAP, for a 1-year contract, with duties including documentation, reporting and community engagement. Applicants should have an educational background in social work or youth services and relevant experience.

Qualifications

• Education and related credentials in social work or youth services.
• Experience in youth homelessness, housing access, or related supports preferred.
• Good written and verbal communication; ability to document services accurately.

Responsibilities

• Provide diversion and case management for youth at risk of homelessness.
• Coordinate shelter access and housing placement as appropriate.
• Develop and apply diversion assessment tools with program staff.
• Maintain documentation and client records in SMIS/EMHWare guidelines.
• Engage with community partners and participate in program reporting.
