Good Shepherd Centres is recruiting for the HOMES Program in Hamilton, Ontario. This role provides supportive housing to individuals who are homeless and living with a mental illness, offering crisis prevention, counselling, and recovery planning support in scattered apartment units across the city.

Qualifications

• Post-secondary education in social work or related field required.
• Minimum five years' experience in mental health in community settings.
• Experience in outreach/mobile work is an asset.
• Knowledge of community resources and determinants of health.
• Recovery-oriented and consumer-driven practice required.

Responsabilités

• Assess, plan, crisis prevention and intervention for tenants.
• Provide supportive counselling and education.
• Facilitate social interaction and community integration.
• Link tenants with community services.
• Provide primary support for recovery planning.
• Occasional work in community and tenant apartments; driving required.
• Adhere to health and safety regulations.
• Willing to assist in a hands-on capacity with tenants.
