Job summary

Community Living Mississauga is seeking a dedicated Service Coordination Worker to support people with intellectual disabilities and their families in Mississauga, ON. Permanent, full-time role coordinating services, referring to community programs, and helping individuals access resources while promoting inclusion, independence and self-determination.

The ideal candidate has a related diploma or a university degree with relevant experience, a valid Ontario driver’s license, First Aid/CPR, and

Qualifications

  • Diploma in social services or related field; or university degree acceptable.
  • Valid Ontario driver’s license and access to a vehicle.
  • Current First Aid/CPR certificate.
  • Proficient with MS Office and computer systems.
  • Excellent interpersonal and communication skills.
  • Experience with service coordination or case management preferred.
  • DEI understanding and ability to support diverse populations.
  • Bilingual English/French or other language considered an asset.

Responsibilities

  • Provide service coordination support based on goals identified by people supported and families.
  • Make referrals to community services and programs to address goals.
  • Keep informed about and utilize available community resources.
  • Support people and families to access services during crises.
  • Promote inclusion, independence and self-determination for people supported.
  • Collaborate with coworkers, community partners, people supported and families to solve problems.
  • Identify/report support situations not meeting Minimum Standards Guidelines.
  • Develop new techniques or approaches to supporting people and families.
  • Support DEI principles in planning with families.

Permanent, Full-Time

Working under the guidance of a Supervisor or Manager, this position is responsible for individualized service coordination support for people who have an intellectual disability and their families.

Description of Duties
  • Provide service coordination support based on goals identified by people supported and their families.
  • Make appropriate referrals to services and programs in the community to address the goals identified.
  • Keep informed about and utilize the resources available in the community.
  • Support people and their families to access support during periods of crisis.
  • Promote community inclusion, independence and self-determination for the people supported.
  • Work collaboratively with co-workers, community partners, the people supported and their families to solve problems and resolve conflicts.
  • Promote the standards set by the organization in terms of Personal Outcome Measures and incorporate these standards whenever possible when planning with families.
  • Identify/Report support situations that do not meet the “Minimum Standards Guidelines” set by the organization.
  • Utilize/Develop new techniques, options or approaches to supporting people and their families.
  • Have an understanding and appreciation of diversity, equity, and inclusion (DEI) principles and expected to support Community Living Mississauga’s commitment to DEI by demonstrating respect for diverse people and contributing to an inclusive culture where everyone can connect, belong, and grow.
Qualifications
  • Social Services Worker Diploma (preferred) or Developmental Services Worker Diploma, Child and Youth Worker Diploma with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
  • Will also consider a University Degree with a minimum of 1 year of experience working or volunteering with people who have an intellectual disability.
  • Must have a valid Ontario Driver’s License (Class G or G2), access to a reliable vehicle and Insurance.
  • Current valid First Aid/CPR certificate required.
  • Proficient with PC computer environment; MS Office (Word, Excel, Outlook, etc.) and other software.
  • Excellent interpersonal and communication skills (verbal, non-verbal, written).
  • Ability to work collaboratively in a team environment and with families as necessary.
  • Good problem-solving and conflict resolution skills.
  • Experience providing Service Coordination, Case Management, or other similar supports would be considered an asset.
  • Understanding of community resources and processes would be considered an asset.
  • Bilingualism in English/French or other language would be considered an asset.
