Job description

POSITION SUMMARY: Under the direction of the Program Supervisor (Jasper), provides comprehensive, continuous, flexible support services on an as-needed basis for frail, elderly tenants living in a community apartment complex.

MAJOR RESPONSIBILITIES:

  • Assesses, plans, implements, records, and evaluates necessary support services based on the tenants' choice and independence level.
  • Promotes and supports self-determination with the tenants.
  • Provides or coordinates 3 meals a day, 7 days a week, appropriate to the tenants' needs and choices.
  • Provides or coordinates personal linen and laundry, including incontinent laundry.
  • Provides or coordinates opportunities for individuals and groups to socialize in the apartment complex and in the community.
  • Provides or coordinates medical and social transportation.
  • Provides or coordinates personal care to the tenants by assisting with activities of daily living including: bathing; eating; dressing; personal hygiene; grooming; toileting and ambulation.
  • Provides or coordinates routine health care such as medication management, routine nail and skin care, health monitoring, and other care tasks as might be agreed upon with the Program Supervisor.
  • Provides or coordinates services for tenants who have behavioral problems requiring ongoing staff support, intervention and supervision.
  • Provides or coordinates the ancillary services related to medical care (e.g. physician, pharmacist, therapy, podiatry); social/recreational opportunities; hospice; and other services needed to support the tenants.
  • Provides or coordinates the household services essential for the health comfort of the tenants (e.g. floor cleaning, dusting, and bed making).
  • Provides emergency response and follow up care 24 hours a day, 7 days a week.
  • Participates in the development and presentation of educational programs and in health teaching to tenants, as required.
  • Maintains required documentation and prepares reports.
  • Liaises with the apartment custodian on matters related to tenants' needs.
  • Liaises and maintains good relationships with tenants, significant others, and formal support providers.
  • Advocates for tenants with community agencies.
  • Performs such other related duties as may be assigned.

QUALIFICATIONS:

Education/Experience:

  • Successful completion of a recognized personal support worker or developmental services worker program; or
  • Successful completion of first year of RPN or RN program

Skills/Abilities:

  • A genuine interest and understanding of the needs of the elderly is necessary
  • Ability to communicate effectively and concisely, both orally and in writing
  • Must be self-directed and sensitive to the needs of the elderly
  • Must be competent within the meaning of the Occupational Health and Safety Act

Assets:

  • Minimum of one year working experience in a long-term care environment

CONDITIONS OF EMPLOYMENT:

  • Must be willing to work shifts, including holidays and weekends
  • Must have successfully undergone a "two step" tuberculosis skin test within the 12 months prior to employment
  • Must have and maintain current First Aid and CPR certifications
  • Must undergo a successful Police Vulnerable Sector Check (PVSC)
