Guelph Community Health Centre is seeking a PSH Support Coordinator to assist residents in permanent supportive housing and related sites with healthcare and social service navigation, care coordination, and advocacy.
You will engage clients with complex needs, coordinate referrals, and work with an allied health team to promote housing stability and well‑being across multiple community sites in Guelph.

The Permanent Supportive Housing (PSH) Program offers a long-term, affordable housing option combined with flexible, voluntary support services to help high-needs individuals experiencing homelessness maintain stable housing and live independently.
Delivered through partnerships among multiple agencies, the program assists residents of the PSH building who face significant challenges such as substance use, mental health issues, and other complexities. It focuses on helping them achieve housing stability and improve their quality of life by providing access to housing, practical assistance, counseling, advocacy, landlord mediation, and eviction prevention.
As a member of the Allied Health Team, the PSH Support Coordinatorworks with vulnerable individuals who areresidentsatthe permanent supportive housing building s at 10 Shelldale, ,as well as Grace Gardens, and the transitional housing program at 65 Delhi Street, who are facing multiple and complex barriers to accessing healthcare through system navigation and direct, one-to-one support.
This position work s at multiple sitesin the community.
