Job summary

Guelph Community Health Centre is seeking a Registered Nurse to join the Allied Health Team. The PSH RN will deliver independent top-of-scope medical care to residents at PSH buildings and other sites, collaborating with a multidisciplinary team to support housing stability and wellbeing.

Responsibilities include trauma-informed primary care, chronic disease management, liaising with physicians and community partners, and providing care across multiple community sites, including Grace Gardens

Qualifications

• Baccalaureate in Nursing.
• Current College of Nurses of Ontario Certificate of Competence.
• Two to five years broad-based practical nursing experience in hospital and community settings and/or public health.
• Experience in community settings independently is an asset.
• Knowledge of social determinants of health, harm reduction, and equity-focused practice.
• Experience with equity-deserving populations.
• Proficient with electronic medical records and documentation practices.

Responsibilities

• Collaborate to provide care for clients with trauma and multiple medications for mental health and chronic conditions.
• Screen, monitor and assess patients using relevant screening tools.
• Complete in-person and virtual primary care visits per scope and guidelines.
• Liaise with physicians, NPs and allied professionals to coordinate client care.
• Provide care across multiple sites including PSH buildings and CHC sites.

The Permanent Supportive Housing (PSH) Program offers a long-term, affordable housing option combined with flexible, voluntary support services to help high-need individuals experiencing homelessness maintain stable housing and live independently.

Delivered through partnerships among multiple agencies, the program assists residents of the PSH building who face significant challenges such as substance use, mental health issues, and other complexities. It focuses on helping them achieve housing stability and improve their quality of life by providing access to housing, practical assistance, counseling, advocacy, landlord mediation, and eviction prevention.

As a member of the Allied Health Team, the PSH RN works with vulnerable individuals who are residents at the permanent supportive housing buildings at 10 Shelldale, as well as Grace Gardens, and the transitional housing program at 65 Delhi Street. The RN will provide independent top-of-scope medical care. They will work as part of an interdisciplinary primary and allied healthcare team to provide health care and wraparound services for residents and clients. This position works at multiple sites in the community.

Principal Job Duties:

Clinical:

• Collaboratively provide care for clients experiencing significant trauma and who may be taking multiple medications for mental health and chronic health conditions
• Screen, monitor and assess patients using appropriate depression, anxiety, chronic pain, addictions and chronic disease management screening and assessment tools
• Complete in-person and virtual primary care visits, according to scope and college guidelines
• Liaise with physicians, nurse practitioners and other allied health professionals and community partners to collaborate about client care
• Work at various locations, including at permanent supportive housing buildings, Guelph CHC sites and in community to provide health care to the priority population

Organizational:

• Participate in all relevant CHC programs, activities, meetings, committees, workshops, training and on-going professional development
• Collaborate with interdisciplinary teams to identify and provide health education priority populations and staff working with priority populations
• Assist in the development, monitoring, and analysis of continuous quality improvement and clinical initiatives
• Adhere to the Guelph CHC Confidentiality Policy, undertaking the responsibility of privacy and confidentiality as it extends to clients, staff, Board, students and volunteers of the Centre

Knowledge, Skills, and Abilities:

• Baccalaureate in Nursing
• Current College of Nurses of Ontario Certificate of Competence
• Two to five years broad-based practical nursing experience in a combination of hospital and community settings and/or public health.
• Experience working in community settings independently is an asset.
• Knowledge and experience working within a social determinants of health approach, including understanding harm reduction philosophies and practices
• Experience and comfort working with equity deserving populations
• High level of computer proficiency including the use of electronic medical records and understanding of documentation best practices.
