Job summary

Guelph Community Health Centre is seeking a PSH Support Coordinator to assist residents in permanent supportive housing and related sites with healthcare and social service navigation, care coordination, and advocacy.

You will engage clients with complex needs, coordinate referrals, and work with an allied health team to promote housing stability and well‑being across multiple community sites in Guelph.

Qualifications

• University degree or college diploma in social work or related discipline, or equivalent experience.
• Three to five years’ experience in a community-based organization.
• Strong interpersonal, active-listening and advocacy skills.
• Familiarity with local social service and health programs and systems.
• Experience with mental health, substance use, complex chronic diseases, and language barriers.
• Understanding of harm reduction.
• Excellent English written and oral communication; second language is an asset.
• Proficiency with computers and software programs.
• Valid Class G Ontario Driver's Licence and access to a vehicle.

Responsibilities

• Act as a resource to clients, assisting them in navigating social service and healthcare systems.
• Assist clients with complex needs to engage in their care plans as outlined by their care team.
• Actively work to engage clients with their primary care providers and community supports.
• Advocate with health and social service providers to ensure clients receive the care they deserve.
• Facilitate internal and external referrals to address needs within the determinants of health.
• Lead case coordination between interdisciplinary team and external partners.
