CarePartners seeks a Manager, Coordination Excellence to standardize coordination practices across Ontario, leading nursing, therapy, and personal support services. You will drive performance improvements and adopt new processes province-wide, reporting to the Director, Coordination Excellence.

You will oversee a team of coordination professionals, partner with clinical and operational leaders, and champions best practices to enhance patient, caregiver, and employee experiences.

Qualifications

• Post-secondary education in Healthcare, Business Administration, Operations Management or Leadership.
• Minimum five years in healthcare operations, coordination, patient services, or related field.
• Minimum three years of leadership experience with coaching and mentoring.
• Experience with cross-functional healthcare teams across Nursing, Personal Support, Therapy, and operations.

Responsabilités

• Lead the standardization and continuous improvement of coordination practices across Ontario.
• Drive adoption of best practices across service lines to promote integrated care.
• Analyze coordination performance data to identify opportunities for improvement.
• Manage the implementation of coordination tools and digital solutions.
• Mentor and develop the Coordination Excellence team and partner with operations.
