Job summary

The Clinical Care Program at CAMH is seeking a full-time, temporary Social Worker 2 for the Downtown East Clinic Outpatient team. You will conduct psychosocial assessments, provide counseling to clients and families, and coordinate social supports within a FACT-based community program for adults with major mental illness.

Work with an inter-professional team, supervise MSW students, and collaborate with community partners.

Qualifications

Master’s of Social Work and 3+ years post-graduate experience.
Registered with Ontario College of Social Workers and Social Service Workers.
Eligible for adjunct faculty appointment with CAMH-affiliated faculties.
Knowledge of FACT model and community mental health.
Outpatient mental health experience is an asset.
Strong organization and communication skills; academic responsibilities possible.

Responsibilities

Conduct psychosocial assessments and provide counseling to clients, families and caregivers.
Engage clients in recovery goals and coordinate social supports.
Apply FACT model in community outpatient setting with inter-professional team.
Liaise with community providers and educate clients and families.
Supervise MSW students and collaborate with university faculties.
Occasional evenings and weekend shifts; shift-work in community setting.
