Job summary

CMHA Toronto, a leading community mental health agency, is hiring a Program Manager for the Flexible Assertive Community Treatment (FACT) Team. The role oversees clinical, operational, administrative, and fiscal aspects of an interdisciplinary group serving adults with serious mental illness and concurrent disorders.

Reporting to the Director of Intensive Community Support Services, you will drive quality, risk management, and partnerships to improve client outcomes and community integration.

Qualifications

  • Degree in Health Sciences, Social Work, Nursing, Occupational Therapy, Psychology, Health Administration, or a related field.
  • Minimum three (3) years of progressive leadership, management, or supervisory experience.
  • Minimum five (5) years of experience in community mental health, healthcare, or human services.
  • Experience leading interdisciplinary teams delivering recovery-oriented services to individuals with serious mental illness and concurrent disorders.
  • Knowledge of recovery-oriented, trauma-informed, culturally responsive, anti-oppressive, and person-centred approaches.
  • Knowledge of community mental health systems, housing, social determinants of health, and FACT or similar models.
  • Knowledge of PHIPA, Health Care Consent Act, OHSA, and quality improvement practices.
  • Demonstrated leadership, communication, relationship-building, budgeting, program planning, and quality improvement skills.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Valid Ontario Driver's Licence and access to a reliable vehicle.

Responsibilities

Program Manager – Flexible Assertive Community Treatment Team (Full Time Permanent)
The Role and Opportunity

CMHA Toronto is seeking an experienced and collaborative Program Manager to lead our Flexible Assertive Community Treatment (FACT) Team. Reporting to the Director, Intensive Community Support Services, the Program Manager is responsible for the clinical, operational, administrative, and fiscal oversight of an interdisciplinary team providing community-based mental health services to adults living with serious and persistent mental illness and concurrent disorders.

Working within a recovery-oriented, trauma-informed, culturally responsive, and person-centred framework, the Program Manager provides leadership to support high-quality service delivery, staff development, system partnerships, client safety, and continuous quality improvement. This role plays a critical part in ensuring services align with FACT principles and support individuals in achieving meaningful recovery, community integration, and quality of life.

Hiring Salary Range : $78,238 - $85,718

Hours of Work: 35 per week

Key Responsibilities

  • Provide leadership, supervision, coaching, and performance management to an interdisciplinary FACT team, fostering a collaborative, inclusive, and high-performing work environment.
  • Ensure service delivery aligns with FACT standards and principles, organizational priorities, and recovery-oriented, trauma-informed, culturally responsive, and evidence-based practices.
  • Oversee day-to-day program operations, including staffing, scheduling, service planning, and resource allocation to support effective and responsive client care.
  • Provide clinical leadership and consultation to support staff in managing complex client situations, risk management, crisis response, and treatment planning.
  • Lead program planning, evaluation, accreditation, quality improvement, and outcome measurement initiatives to promote service excellence and continuous improvement.
  • Monitor compliance with organizational policies, professional standards, legislative requirements, documentation standards, and funder expectations.
  • Develop and maintain strong partnerships with hospitals, primary care providers, housing organizations, community agencies, and other stakeholders to support coordinated service delivery and system collaboration.
  • Support budget planning, reporting, operational activities, and service development initiatives that enhance client outcomes and program effectiveness.
  • Represent CMHA Toronto on internal and external committees, planning tables, and community initiatives as required.
  • Ability to work independently, within the community working with population with complex needs
Experience & Education Required

CMHA Toronto’s ideal candidate will have:

  • Degree in Health Sciences, Social Work, Nursing, Occupational Therapy, Psychology, Health Administration, or a related field; registration with a regulated profession is considered an asset.
  • Minimum three (3) years of progressive leadership, management, or supervisory experience.
  • Minimum five (5) years of experience in community mental health, healthcare, or human services.
  • Experience leading interdisciplinary teams and supporting staff in delivering recovery-oriented services to individuals with serious mental illness and concurrent disorders.
  • Knowledge of recovery-oriented, trauma-informed, culturally responsive, anti-oppressive, and person-centred approaches to service delivery.
  • Knowledge of community mental health systems, supportive housing, social determinants of health, and FACT or similar community-based service models.
  • Knowledge of relevant legislation and professional standards, including PHIPA, the Health Care Consent Act, Occupational Health and Safety legislation, and quality improvement practices.
  • Demonstrated leadership, communication, relationship-building, conflict resolution, budgeting, program planning, and quality improvement skills.
  • Strong organizational, analytical, and decision-making skills, with the ability to balance competing priorities in a complex environment.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Valid Ontario Driver's Licence and access to a reliable vehicle.
