Job summary

Community Living BC is seeking a Facilitator in Prince George. The role is auxiliary/temporary (70 hours bi-weekly) with a hybrid work model (3 days/week in office after probation).

You will work with individuals with developmental disabilities and families to uncover needs, strengths, and goals, turning them into meaningful opportunities. Responsibilities include coordinating services, supporting access to funded programs, and collaborating with partners to build person-centred plans.

Qualifications

• Bachelor of Social Work or equivalent degree.
• Experience in Community Living with individuals with developmental disabilities and families.
• Equivalent combination of education and experience considered.

Responsibilities

• Support and facilitate access to generic services and CLBC-funded services.
• Assist individuals and families to identify activities and resources to reach goals.
• Provide information about informal supports, CLBC services, and peer/family supports.
• Collaborate with other service providers to respond to crises and resolve problems.
• Coordinate services and implement individual support plans.
