Job summary

AgeCare in British Columbia is seeking a dedicated Social Worker to support residents and their families. You will assess needs, assist with the move‑in process and transitions, coordinate with POAs, and help ensure a comfortable, person‑centered approach to care.

In this role you will participate in the residence’s CQI program, evaluate service effectiveness, update policies, and prepare required reports for the residence or corporate office.

Qualifications

• Bachelor's or Master's Degree in social work.
• Minimum one year of relevant experience, preferably in a health care setting.
• Experience in long-term care is preferred.

Responsibilities

• Assist residents and families with move-in processes and transitions.
• Support CQI initiatives and contribute to policy updates.
• Prepare reports for the residence or corporate office as required.
• Provide assessments and access to community resources.

Working in long-term care is more than just a job; it is a calling for many of us.The Opportunity:The Social Worker assists residents and their families by providing support and assessment for residents, related but not limited to, move-in process, transitions, finances, concerns and decline in health as well as assistance accessing community resources.What Is In It For You:• Assists the Director of Care and Business Manager with the process of moving new residents into the home by coordinating with incoming residents and their Power of Attorney (POA) and providing support with aspects of the move-in process that require the involvement of the Social Service Worker.• Participates in the residence’s Continuous Quality Improvement (CQI) program.• Evaluates the effectiveness of Social Service Work services on an annual basis and provides input to update policies and procedures that impact the Social Service Work practice.• Prepares and submits reports to the residence and/or corporate office as required.What You Will Need:• Bachelors or Master's Degree in social work.• Minimum one-year relevant experience preferably in a health care setting.• Previous experience in a long-term care setting is preferred.Salary Range: $42.27 - $52.81 per hourSalary placement based on qualifications and experience.
