AgeCare is seeking a Social Worker to join an interdisciplinary care team in Edmonton. The role supports residents and families through psychosocial assessments, planning, counselling, and referrals, reporting to the Executive Director.

You are registered with ACSW, hold a Bachelor of Social Work, and have experience with geriatric populations. This on-site, part-time position emphasizes patient-centered care, confidentiality, and collaboration with healthcare professionals.

Qualifications

Responsibilities

• Complete psychosocial assessments and support residents and families.
• Advocate for residents' rights, dignity, autonomy and quality of life.
• Provide education, resources, referrals, and assistance related to community services, healthcare and legal matters.
• Educate and guide staff on psychosocial and legal matters within scope of practice.
• Collaborate with the team to identify psychosocial risks and contribute to care planning.
• Complete Social Work documentation and assessments within timelines.
• Pre-screen admissions and identify decision-makers and funding considerations.
• Participate in Care Conferences and support communication and conflict resolution.
• Assist with legal documentation guidance when needed.
• Participate in interdisciplinary meetings and quality-improvement initiatives.

Work Location: AgeCare South Terrace Address: 5905 112 St NW, T6H 3J4 - Edmonton, AB FTE: 0.50, 20 hours week Employment Type: Regular Part-Time, On site

The Opportunity

The Social Worker reporting to the Executive Director is a key member of the interdisciplinary care team, collaborating on the delivery of care to meet resident and family needs. The Social Worker provides a range of services based on a comprehensive psychosocial assessment, including planned interventions, adjustment support, counselling, education, consultation, referrals, and assistance with financial and legal matters.

What You Will Do

• Complete comprehensive psychosocial assessments and provide ongoing support to residents and families related to adjustment, grief and loss, changing health needs, family dynamics, and other life transitions.
• Advocate for residents' rights, dignity, autonomy, preferences, and quality of life, while maintaining confidentiality and professional boundaries.
• Provide residents and families with education, resources, referrals, and assistance related to community services, healthcare, financial matters, legal documentation, and end-of-life planning.
• Provide education and guidance to staff on relevant psychosocial and legal matters within the Social Worker's scope of practice.
• Collaborate with the interdisciplinary team to identify psychosocial risks, develop appropriate interventions, and contribute to individualized care planning, transitions, transfers, and discharge planning.
• Complete required Social Work documentation, MDS assessments, and Care Plan sections, ensuring information is accurate, current, and completed within required timelines.
• Conduct pre-screening of potential resident admissions, including identifying decision-makers, reviewing relevant legal documentation, and identifying financial or funding considerations.
• Participate in Care Conferences and support communication, conflict resolution, and mediation between residents, families, and the care team when needed.
• Assist residents and families when required legal documentation is not in place by providing information, appropriate forms and resources, and guidance regarding next steps.
• Participate in interdisciplinary meetings, committees, and quality-improvement initiatives that enhance resident well-being, quality of life, and the overall resident and family experience.

What You Bring

• Current registration with the Alberta College of Social Workers (ACSW); proof of registration and a valid practice permit must be provided annually.
• Bachelor of Social Work degree from a recognized institution.
• Experience working with geriatric populations is an asset.
• Ability to work effectively within a multi-disciplinary team setting.
• Self-directed with the ability to initiate and manage departmental activities.
• Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
