Cencora is seeking a Nurse Case Manager in Canada to support timely patient access, treatment progression, and continuity of care. This role helps coordinate complex care across providers, pharmacies, payers, and internal partners, with a focus on reducing barriers and improving outcomes.

You will collaborate with healthcare teams to optimize patient pathways and ensure seamless transitions, while adhering to local policies and quality standards.

Qualifications

• Bachelor's degree in nursing or related field or equivalent experience.
• 4+ years in care management or related field.
• Certification in case management or equivalent preferred.

Responsibilities

• Plans and manages complex care coordination cases to support timely patient access and continuity.
• Assesses, plans care, and facilitates transitions addressing clinical, social, and access barriers.
• Coordinates with providers, pharmacies, payers, and partners to resolve access barriers and maintain continuity.
• Analyzes case trends and workflow to identify root causes and suggest improvements.
• Develops job aids and templates to improve consistency and reduce rework.
• Guides less experienced team members on complex case handling and documentation expectations.
