Job summary

Cencora Canada is seeking a Nurse Case Manager for a 12-month contract to support patient enrollment, therapy coordination, case progression, and access to prescribed treatment.

You will review patient files, coordinate with healthcare providers, pharmacies and clinics, audit activities, and ensure documentation quality and regulatory compliance while guiding patients and teams through complex care pathways.

Qualifications

• Diploma in health care or related field; preference for Bachelor's degree.
• Licensed Nurse (RN/RPN/LPN) with active provincial registration.
• Experience in care coordination and case management.

Responsibilities

• Review patient files and treatment history to confirm readiness for case action.
• Coordinate enrollment, therapy coordination, and case progression.
• Audit case activity and documentation quality to identify gaps.
• Evaluate therapy access barriers and escalate as needed.
• Provide clear direction to patients, providers, and internal teams.
• Coordinate with providers, pharmacies, clinics, and support teams.
• Uphold reporting and documentation standards per regulatory requirements.
