Job summary

Sienna Senior Living in Etobicoke is seeking a Resident & Family Experience Coordinator (Social Services Worker) to join the Long Term Care team. You will be the main point of contact for residents and families, coordinating care and services to enrich daily health and well-being.

This role focuses on hospitality, service excellence, and partnership with leadership to address concerns, conduct care conferences, and lead satisfaction surveys.

Qualifications

• Completed a College/University degree.
• Provincially registered Social Services Worker.
• 3 years’ experience in healthcare, social services or related sector.
• Customer-focused with attention to detail; able to multi-task.
• Legally eligible to work in Canada or have a work permit.

Responsibilities

• Provide care and service coordination to residents and families.
• Lead the resident move-in, transfer, and move-out process.
• Provide social services work to residents living in long-term care.
• Coordinate resident and family satisfaction surveys and develop actions to improve experiences.
• Coordinate and facilitate resident care conferences.
• Engage community guest speakers for in-service sessions related to long-term care.
• Support leadership to improve resident care and services.
• Contribute to quality improvement plans to increase quality of care and reduce risk.
