Job summary

Alberta Health Services is seeking a Registered Nurse to join the Short Term Intervention Team delivering high-quality home nursing across the Southwest Home Care program. You will assess needs, plan care and provide clinically skilled services in clients' homes.

You will work shifts across days, evenings and weekends, with on-call opportunities, and collaborate with the broader health team to ensure safe, patient-centred care for diverse populations.

Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with the CRNA.
• Current Basic Cardiac Life Support - Health Care Provider certification.

Responsibilities

• Assess needs, deliver therapeutic nursing services in clients' homes.
• Coordinate resources, set care goals and evaluate outcomes.
• Provide case management and direct client care meeting clinical standards.
• Act as consultant to team; plan to address unmet nursing needs with resources.
• Triaged and supported clinics; perform community home visits with cardiac, IV therapy and wound care.
