Job summary

Alberta Health Services is seeking a Registered Nurse to deliver home and community care in Calgary. You will perform nursing assessments, coordinate care and provide direct nursing services in clients’ homes or alternate settings, following a client-centered strengths-based framework.

You will collaborate with clients, families and an interdisciplinary team to adapt plans, ensure safety and maintain quality care while managing time and reporting to the Home Care Manager.

Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Responsibilities

• Provide nursing assessments, case coordination and direct care.
• Coordinate and provide quality care to clients in the community.
• Collaborate with client/family and interdisciplinary team to adjust services as needed.
• Report to the Home Care Manager and ensure services align with patient goals.
