Job summary

Yee Hong Centre for Geriatric Care seeks an Activation Worker to plan and conduct daily activation, recreational and social programs for residents of the LTC Home, reflecting a resident-centred approach to enhance quality of life.

The role involves assessing residents, supervising volunteers, and collaborating with interdisciplinary teams to deliver meaningful activities within budget. Strong English/Chinese communication and CPR/food-safety certifications are expected.

Qualifications

• Experience in a social services or long-term care setting is an asset.
• Experience and skills in facilitating activation and therapeutic recreation programs, preferably working with the geriatric population.
• Creativity and innovativeness in developing meaningful programs for clients within a tight budget.
• Superb customer service skills. Demonstrated initiative to support the clients and team in a positive and efficient manner.
• Current CPR Certificate and Food Handler Safety Certificate.
• Satisfactory Police Vulnerable Sector Screen.
• Ability to communicate in English and Chinese.

Responsibilities

• Plans and conducts daily activation, recreational and social programs for residents.
• Assesses residents upon admission and quarterly; monitors care plans.
• Supervises volunteers and collaborates with students and interdisciplinary team members.
• Maintains resident records and works with staff to provide quality care.
• Sho ps for cost-effective program supplies and tracks fees and expenditure.
• Provides information and education to residents and families (hand hygiene workshop).
• Participates in in-service programs and professional development.
• Keeps up-to-date with developments and best practices in the field.
• Other duties as assigned.
