Job summary

Yee Hong is seeking an Activation Worker to plan and conduct daily activation, recreational and social programs for LTC Home residents. This role focuses on resident-centered approaches to improve quality of life and requires collaboration with a multidisciplinary team.

The position emphasizes developing innovative activities within budget, supervising volunteers, maintaining records, and supporting families with education on health practices. Proficiency in English and Chinese is valued.

Qualifications

  • Diploma or equivalent in recreation field or therapeutic recreation.
  • Experience in social services or long-term care is an asset.
  • Ability to develop meaningful programs within a tight budget.
  • Excellent customer service with initiative to support clients and team.

Responsibilities

  • Assess residents on admission and quarterly, and monitor care plans.
  • Plan and run regular and seasonal activities to improve quality of life.
  • Supervise volunteers and mentor students and interdisciplinary staff.
  • Maintain resident records and prepare cost-efficient program supplies.
  • Provide information and education to residents and families; engage in professional development.
  • Ensure safe, hygienic activity environments and proper documentation.

Job description

Under the supervision of the Activation Manager, the Activation Worker plans and conducts daily activation, recreational and social programs for residents of the LTC Home that reflects the organization's resident centred approach to improve and maintain the residents' quality of life in the Home.

Responsibilities
  • Conducts timely and accurate assessment of residents upon admission and on a quarterly basis. Prepares and monitors effectiveness of care plan
  • Plans and implements innovative regular and seasonal recreational and social activities to improve residents' quality of life, taking into consideration their feedback, needs and preference, and ensures activities are conducted in a safe, hygienic and supportive environment, including one-on-ones, small group and large group activities
  • Assists the Activation Manager in supervising volunteers who assist or facilitate activation programming
  • Mentors and works effectively with students and interdisciplinary team members
  • Maintains and updates resident records
  • Works with staff of other disciplines in the LTC Home in providing quality care to LTC Home residents
  • Shops for cost effective and appropriate program supplies and maintains accurate record of fees and expenditure
  • Assists the Activation Manager in providing information and education to residents and their family members (hand hygiene workshop)
  • Participates in in-service programs and professional development
  • Keeps up-to-date with developments and best practices in the field
  • Other duties as assigned
Qualifications
  • Diploma in Recreation and leisure studies, therapeutic recreation, kinesiology, or equivalent
  • Experience in a social services or long-term care setting an asset
  • Experience and skills in facilitating activation and therapeutic recreation programs, preferably working with the geriatric population
  • Creativity and innovativeness in developing meaningful programs for clients within a tight budget
  • Superb customer service skills. Demonstrated initiative to support the clients and team in a positive and efficient manner.
  • Computer literacy. Knowledge of Chinese word processing, an asset
  • Current CPR Certificate
  • Current Food Handler Safety Certificate
  • Satisfactory Police Vulnerable Sector Screen
  • Ability to communicate in English and Chinese
