Job summary

Yee Hong Centre for Geriatric Care is seeking a Program Assistant (Casual) in Toronto, Ontario to plan activities and provide quality services to clients of the Day Program. The ideal candidate will have a diploma in Social Work or related fields, alongside significant experience in geriatric rehabilitation.

The role demands excellent customer service skills and the ability to work both independently and as part of a team. Responsibilities include ensuring optimal operation of activities and meeting clients’ physical and emotional needs.

Qualifications

• Experience in geriatric rehabilitation is essential.
• Recent certification in C.P.R. required.
• Recent satisfactory Police Vulnerable Sector Screen result.

Responsibilities

• Participates in the planning of appropriate activities for clients of the program.
• Provides timely support, information, referral, and outreach to clients.
• Maintains up-to-date and accurate files, records, and statistics on the program.

Skills

Fluency in English

Program Assistant (Casual)

Under the direction of the Program Manager/Program Coordinator, plans or participates in the planning of appropriate activities and providing high quality services to clients of the Day Program.

Qualifications

• Diploma in Social Work or Social Services, health sciences, activation, or equivalent
• Experience in geriatric rehabilitation is essential
• Excellent customer service skills
• Ability to work independently and contribute effectively as a team player
• Computer literacy in Microsoft Office Suite
• Recent certification in C.P.R.
• Recent satisfactory Police Vulnerable Sector Screen result
• Fluency in English and ability to converse in Chinese
• Must be able to work across different Yee Hong sites as needed
• Successful applicants must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.

Responsibilities

• Participates in the planning of appropriate activities for clients of the program
• Implements and ensures the smooth operation of the activities in a safe, hygienic, and supportive environment
• Provides timely crisis intervention or behaviour management and ensures clients’ physical, social and emotional needs are appropriately met
• Provides timely support, information, referral, placement, and outreach to clients and volunteers to meet their social, recreational, and developmental needs
• Works closely with clients and caregivers to ensure appropriate design and ongoing monitoring of the care plan/program plan
• Ensures appropriate assessment, placement, matching, and support for volunteers
• Provides effective guidance, training, and support to volunteers in the program
• Maintains up-to-date and accurate files, records, and statistics on the program
• Maintains accurate records of fees and expenditures
• Complies with risk management policy for health and safety and infection control
• Keeps up‑to‑date with developments and best practices in the field

Salary Range

$22.52 - $26.49 per hour.
