Job summary

St. Joseph’s Healthcare Hamilton is seeking a highly motivated Registered Nurse to join the Renal Transplant Clinic as a Primary Care Nurse.

You will coordinate patient care from referral to post-transplant follow-up, educate patients and families, and act as the main contact for patients, families, and providers. The team includes RNs, nurses practitioners, physicians, pharmacists, social workers, dietitians, and admin staff, working together to support transplant patients and donors.

Qualifications

  • Current registration with the College of Nurses of Ontario required.
  • Minimum of two (2) years of nephrology, dialysis, or transplant nursing experience required.
  • Demonstrated leadership in renal or related nursing (Charge Nurse/Team Lead).
  • Completion of nephrology or transplant education (e.g. courses/certificate, cNeph).
  • Active participation in or leadership of committees; education presentations.
  • Current BCLS/HCP certification required.
  • Minimum of one (1) year of transplant experience preferred.
  • RNAO membership preferred; CPI and/or GPA training preferred.

Responsibilities

  • Coordinate care from referral through transplant journey, including listing and post-transplant care.
  • Educate patients and families about the transplant journey.
  • Serve as key contact for patients, families, referring physicians, and providers.
  • Collaborate with multidisciplinary team and partner organizations.
