Job summary

St. Joseph's Health Centre, Toronto is seeking a Registered Practical Nurse to deliver holistic, evidence-based nursing care in a changing hospital environment.

You will advocate for patient and family needs, collaborate with the inter-professional team, and ensure safe, effective care aligned with Ontario standards. You will assess, plan, implement and evaluate patient care, document accurately, and contribute to transitions and education.

Qualifications

• Successful completion of a Practical Nursing Diploma Program.
• Currently registered as a Registered Practical Nurse with the College of Nurses of Ontario.
• Current BLS/CPR certificate required.
• Palliative care courses are an asset.
• Recent acute inpatient medicine clinical experience and/or palliative/oncology experience required.

Responsibilities

• Provide holistic, evidence-based, patient-centered care in line with College of Nurses of Ontario standards and Unity Health Toronto policies.
• Coordinate and communicate patient care activities to optimize outcomes.
• Engage in teaching and mentoring within the inter-professional team.
• Document observations and care provisions accurately for effective communication.
• Contribute to discharge planning and care transitions while advocating for appropriate settings.
