Responsibilities

• Review assigned online service-change forms.
• Check account reference and required fields using the proper checklist.
• Note the exact requested change and any preferred date.
• Ask for a missing detail where the process permits it.
• Attach a corrected form to the existing request.
• Route a complete submission to the authorized team.
• Keep approval and receipt statuses distinct.
• Protect private account information in the assigned system.
