Job summary

Sentrex Health Solutions is seeking a Care Navigator to manage enrollment, reimbursement, and service coordination for patients in the PSP. This remote role supports physicians, clinics, and patients with drug coverage and timely access to therapies.

The Care Navigator will communicate with insurers, physicians, and patients, ensuring accurate documentation, eligibility assessments, and coordinated services. A healthcare background and bilingual English/French are assets.

Qualifications

• Post secondary education in healthcare is an asset.
• Bilingualism (English and French) is an asset.
• Experience with reimbursement billing and the appeals process is an asset.
• Knowledge of private and public reimbursement structures and systems is an asset.
• Must be able to work from home in a quiet private space.

Responsibilities

• Act as the primary contact for all patient-related elements of the PSP.
• Ensure timely enrollment of patients into the Patient Support Program.
• Provide telephone support and adherence assistance.
• Collaborate with insurer and physician on documentation for coverage.
• Review patient charts and assist physicians with coverage documentation.
• Provide therapy guidelines and program education to manage expectations.
• Coordinate information and conduct patient financial eligibility assessments.
• Maintain CRM patient/clinic information and ensure timely service coordination.
• Follow up to secure coverage and resolve issues as they arise.
