Job summary

Wesley Halton Housing is seeking an Intensive Case Manager to support housing services in Halton. You will work under the Halton Housing Services Manager, assisting with intake and daily administrative tasks for a client-centered, harm-reduction housing first approach.

The role requires a post-secondary credential in Human Services and OCWS/OCSWSSW registration, plus experience with homelessness, mental health, and addictions.

Qualifications

• Post-secondary degree/diploma in Human Services or related field.
• Registration and member in good standing with the OCWS/OCSWSSW is required.
• Experience with adults and families facing mental health/addictions and homelessness.
• Ability to work flexible shifts including day, evening, weekends, and 24-hour intake.
• Valid driver’s license and access to a vehicle is an asset.

Responsibilities

• Assist with intake supports and day-to-day administrative tasks.
• Place individuals/families into permanent housing and coordinate supports.
• Screen participants for service needs and assist transition to stable housing.
• Complete housing applications and maintain active status as needed.
• Refer participants with domestic violence concerns to appropriate programs.
• Provide move-in support and neighborhood orientation.
• Maintain communication with housing providers, partners, and landlords.
• Report to CAS if child abuse/neglect is suspected.
• Conduct home/shelter visits to deliver life skills and budgeting support.
• Maintain accurate record-keeping and participate in 24-hour intake when needed.
• Other duties as assigned.

Are you the one we’re looking for?

We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment to our clients, families, and each other.

Reporting to the Manager of Halton Housing Services, this position will assist in providing intake supports and carrying out day-to-day administrative services. Wesley Halton Housing offers services to the homeless in Halton Region, supporting homeless families and individuals in a transitional program on-site. Services are based on a client-centered, residential harm reduction, housing first approach to service delivery.

This position is unionized, full-time, temporary. The contract is until July 1, 2026. The rate of pay is $24.38.

Key Responsibilities

1. Works with assigned families and individuals in a housing program that works towards a housing plan. This includes maximizing income, filing taxes, obtaining an ID, and connecting with community support.
2. Responds to calls and referrals from community agencies in accordance with the case load.
3. Places individuals and families into permanent leased units, securing appropriate housing based on individual needs.
4. Assess and screens participants’/families’ needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources.
5. Completes Social & Community Housing applications (HATCH) to submit to the Region, including all required documents for approval, OR ensures existing application remains active.
6. Assess clients for any recent, current, or ongoing experience with domestic violence and make referrals to appropriate programs.
7. Assist participants with their move into independent accommodations and provide neighborhood orientation.
8. Maintain regular, effective communication with social housing providers, administrators, community partners, and private landlords to ensure an integrated approach that will provide the best support possible for clients to maintain housing.
9. Report to the appropriate child welfare agency (CAS) if child abuse and/or neglect is suspected or witnessed.
10. Conducts regular visits with participants in their homes or shelter to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration, and community building, grocery shopping, cleaning, etc.
11. Maintain appropriate and accurate record-keeping practice, as per the expectations of Wesley.
12. Supports approximately 1 shift per month of 24-hour intake; flex time is available for additional hours worked.
13. Other duties as assigned.

What you will bring

1. Post-secondary degree/diploma in Human Services or related field.
2. Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
3. Experience working with adults and families with mental health and addictions issues and the homeless and providing non-judgmental support to them.
4. Must be able to work flexible shifts, including day, evening, and weekend hours as well as 24-hour intake responsibilities.
5. Must be able to meet travel expectations of the position; a valid Driver’s license and access to a vehicle is a definite asset.
