Job summary

CarePartners is seeking RNs and RPNs to join our community nursing team, delivering patient-focused care remotely and in homes across Ontario. The role offers flexible hours, on-call rotation, and opportunities for advancement within a supportive environment.

You will work across home, clinic, retirement home or school settings, providing skilled nursing services to patients while maintaining dignity and privacy. Strong English communication, valid license, and immunizations are required.

Qualifications

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree/diploma.
• Current College of Nurses of Ontario (CNO) registration in good standing.
• A valid driver’s license and vehicle as regular travel is required.
• Excellent English verbal and written communication skills.
• CPR Certification and immunization records.
• Vulnerable sector check and a clear background check.

Responsibilities

• Developing and maintaining an effective Care Plan with respect to Community Health Services.
• Working with patients (both in-person and virtually) to provide exceptional nursing services.
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school.
• Communicating and liaising with patients, family members and others within the interdisciplinary care team.
• Performing other clinical duties as required.

Wages

• For Registered Nurses (RN)- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RN per general visit range is $30.20 to $44.58 and the RN hourly range is $30.80 to $45.48. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
• For Registered Practical Nurses (RPN) - This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RPN per general visit range is $24.07 to $29.60 and the RPN hourly range is $24.07 to $29.60. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patients (both in-person and virtually) to provide exceptional nursing services
